Policies & Fees

We accept most health insurance plans, and workers compensation or NY no-fault cases. If you have an unusual or out-of-state health plan, please call your insurance company for a coverage determination.

It is important to understand how your health plan works, what services it covers, and what it requires of you. If you have any questions about your coverage or obligations, please call your health plan. We must follow the requirements of your health plan and cannot treat patients without required co-payments and/or referrals at the time of your appointment.

What to Bring to Your Office Visit:

  1. PATIENT INFORMATION FORMS - Please fill out any forms you received from our office before your appointment and bring them in with you.
     
  2. INSURANCE CARD(S) - If we are unable to verify your insurance at checkin, your appointment will need to be rescheduled.
     
  3. Any CO-PAYMENT, CO-INSURANCE, OR DEDUCTIBLE required by your health plan - These are due at checkin for your appointment. If you are not prepared with your required payment, your appointment will need to be rescheduled.
     
  4. A REFERRAL to Our Office (if required by your health plan). Your health plan may require you to get a referral from your primary care physician (PCP) before your first visit to our office. If you are unsure, call your health plan to find out. Please understand it is YOUR responsibility to get a REFERRAL from your PCP before you come to our office. If you do not have a needed referral before your visit, your appointment will need to be rescheduled.
     
  5. MEDICAL INFORMATION RELATED ONLY TO THE PROBLEM you are seeing us for.
     
    • Get related medical records and lab/radiology reports from your PCP (referring doctor). Bring copies to your appointment or ask that they be FAXED to 677-5008 or 677-5513.
       
    • Get related radiology films (mammograms, CT scans, MRI’s, MRA’s, angiograms, etc.) from the facility where they were performed. Call that facility at least 24 hours before you plan to pick up your films so they can have them ready for you.

Workers Comp or No-Fault Claims:
Bring in your health insurance card and obtain a referral (see above) so your health plan will cover your bills if your claim is disputed.

Fees Not Covered by Insurance:

  • Missed Appointment Fee (w/o providing us 24 hours notice) - $25
     
  • Returned Check Fee - $25
     
  • Copying records to be released to the patient - $.75/page
     
    Processing Time: 5 business days There is no charge if we send your records directly to another physician. Copying done on behalf of an attorney or insurance company may incur additional charges. A postage charge will be added at cost.
     
  • Form Prep. Fees {Your FIRST form is free of charge} - $10 each additional
     
    Processing Time: 10 business days We complete forms as a courtesy to our patients - please allow 10 business days to complete your forms.
     
    ** We do not charge to prepare handicap-parking renewals or public assistance forms. **

You will be asked to pay $50 at check-in in any of the following situations:

  • you do not have current insurance from a company we contract with;
  • we are unable to verify your insurance coverage;
  • you are responsible to pay an insurance deductible;
  • you have an unpaid patient account.
